HCSL will utilize its own Roster form for Fall 2009 games.  It will combine the pictures used in ID cards and a roster in a single form.  The document is a MS Word fill-in form that needs to be completed for each player and official for the team.  Pictures used for other identification can be pasted into the attached form.  Additional instructions to follow.

Once completed, the form and a copy of a birth certificate for each player will need to be submitted to HCSL for approval prior to the first game.  Once approved, this form must be available and presented prior to each game.
